DESCRIPTION:The flu shot is covered at 100% with a $0 co-pay for any individual that is covered by Medicare or Medicaid.  Most other insurances also cover at 100%  with a $0 co-pay.  Please bring your insurance card or a copy and a completed consent form with you.  If you would like to call ahead and have the flu vaccination run on your insurance prior to the event to ensure your coverage\, please call the Meijer Pharmacy at 859-264-3064 and speak to an associate who can help you with that.\n\nYou may pick up a consent form at Clark County Community Services\, 30 Taylor Ave\n\n*Consent forms and Insurance copies can be obtained that day in the event you are unable to \n\n   Bring them with you.\n\n \n\nWe can also have other vaccinations available that day if anyone voices a desire for those.  \n\nTDAP (for expectant mother's and families)\n\nSHINGLES  - (most insurance covers at age 50 or 60)\n\nPNEUMONIA -  (seniors 65+)
